8th Grade Parents,
Just a reminder of the health document requirements for your child entering 9th grade. Each student is required to have a physical exam by September 15th that is dated within 1 year prior to August 14, 2024 or after. Failure to complete this requirement by September 15th will result in your child being excluded from school until the requirement is met. The exam must be completed on the required “Illinois Certificate of Child Health” form. Please note an IESA/IHSA sport physical form MAY NOT be used as the required school health exam. Parent signature and completion of the Health History section is required on this form!
